Billing Office hours are Monday through-Friday 8:30 AM to 4:00 PM.

Our physicians accept most major insurance carriers. Please check with your insurane carrier if you need to verify that your insurance will be accepted.

Payment for office services, co-pays, and deductibles are expected at the time of service.

If you are unable to pay for your services, we will be happy to re-schedule your appointment. We will file your insurance claim for you as a courtesy if you are covered by one of the plans with which our physicians participate with.

If our physician does not participate in your plan, or you do not have insurance coverage, your payment in full for medical services will be due upon discharge from the clinic. The Clinic accepts cash, checks, and most credit cards. Please understand that your insurance policy is a contract between you and your insurance carrier. You should direct any questions or complaints regarding coverage or amount of payment to your insurance carrier directly.

If you are currently a member of one of the HMO/PPO's that we are contracted with, please present your membership identification card to our receptionist. Your medical services will be billed directly to the insurance company. All co-pays must be made prior to your visit. Any service that is denied as "non-eligible" will be your responsibility. If you are a member of an HMO/PPO not listed, please check with our billing department to see if the clinic is a member of your particular plan. It is your responsibility to identify the insurance under which you are covered. Any charges incurred because of failure or improper identification of HMO/PPO membership will be your responsibility.

Midwestern University Clinic cannot become involved with third party payers for medical services. If you are a party to litigation such as an auto accident, homeowner's injury, divorce, child custody, guardianship, etc., the clinic Clinic will not bill any of these parties or wait for payment from a settlement. Your bill for medical services must be paid in full with each visit. You will be given an itemized receipt as documentation of your visit / payment to present for any reimbursement from your litigation process.

In respect to minors of divorced parents, regardless of who retains custody of the minor child, or who maintains insurance coverage, all co-pays and any or all deductibles must be paid at the time of service according to the insurance plan coverage.

Dental Institute

Dental Institute insurance information will be available in late Spring, 2010, when the clinic opens.

Eye Institute

Eye Institute insurance information will be available in late Spring, 2010, when the clinic opens.
