Our physicians accept most major insurance carriers. Please check with your insurane carrier to verify that your insurance will be accepted.

Payment for office services, co-pays, and deductibles are expected at the time of service.

If you are unable to pay for your services, we will be happy to re-schedule your appointment. We will file your insurance claim for you as a courtesy if you are covered by one of the plans with which our physicians participate with.

If our physician does not participate in your plan, or you do not have insurance coverage, your payment in full for medical services will be due at time of service. The Clinic accepts cash, checks, and most credit cards. Please understand that your medical insurance policy is a contract between you and your insurance carrier. You should direct any questions or complaints regarding coverage or amount of payment to your insurance carrier directly.

If you are currently a member of one of the HMO/PPOs that we are contracted with, please present your insurance identification card upon check-in at the front desk. Your medical services will be billed directly to your insurance company. All co-pays must be paid prior to your visit. Any service that is denied as "non-eligible" will be your responsibility. If you are a member of an HMO/PPO that we are not contracted with, please check with our billing department to see if the clinic is a member of your insurance plan. It is your responsibility to disclose the insurance carrier, under which you are covered. Any charges incurred due to failure or improper identification of HMO/PPO membership will be your responsibility.

In respect to minors of divorced parents, regardless of who retains custody of the minor child, or who maintains insurance coverage, all co-pays and any or all deductibles must be paid at the time of service according to the insurance plan coverage.

There is an initial fee to evaluate you as a potential teaching case after your screening appointment. This initial fee includes an examination and x-rays. Payment is due at the time service is provided. For your convenience, we accept personal checks with valid identification, cash, MasterCard, Visa, Discover, and American Express. There will be an additional $30.00 charge for returned checks. Patients will not receive treatment until all fees and outstanding charges are paid in full. If you are unable to pay for your services, we will be happy to re-schedule your appointment.

Fees are expected and due at time of service. However, multi-step appointment services (e.g. crowns, bridges, complete and partial dentures, implant procedures or root canal procedures) require payment of 50% of the fee at the time of the start of treatment. The remaining balance for that treatment is due at the beginning of the appointment when the restoration is completed or the appliance is delivered.

As a courtesy, we will provide a copy of the dental claim form for you to submit to your insurance company for reimbursement, once services are paid in full.

We are here to service the community, and because of such we do have programs to assist the public in times of financial hardship. Please contact the Assistant Manager of Patient Accounts at your clinic regarding the sliding fee scale.
